Important details:
- Submit forms as early as possible, ideally before school starts, since the clearance process can take time.
- The packet only needs to be completed once per school year, as long as the Health Certificate is unexpired.
- Once you submit, please forward the Seamless Docs confirmation to Murch.Athletics@k12.dc.gov. This helps us track who has submitted and helps Murch assist in the process of getting packets cleared
